Why the 8D Methodology is Different

  1. The Occurrence: Why did the problem happen in the first place?


  1. The Escape Point: Why did our quality system fail to catch the problem before it reached the customer?

D0: Plan or Preparation


Before you even form a team, you must protect the customer. If you haven't implemented an Emergency Response Action (ERA), you aren't doing an 8D—you're just filling out a form.


D1: Initiate Project Team


Assemble a cross-functional group with the technical expertise and the authority to implement changes.


D2: Describe the Problem


Use the "5W2H" approach (Who, What, Where, When, Why, How, How Many) to define the problem in quantifiable terms.


D3: Interim Containment Actions (ICA)


Implement temporary "band-aids" to isolate the problem from the customer until permanent changes are verified.

D4: Identify Root Cause and Escape Point


Utilize tools like the 5 Whys and Fishbone Diagrams to identify the actual cause of the failure.

D5: Develop and Verify Solution (PCA)


Select the best solution and run "mini-tests" to ensure the fix actually works without creating new problems.


D6: Implement and Validate PCAs


Roll out the solution and monitor the results over time to ensure the defect has been eliminated.


D7: Prevent Recurrence


Modify management systems, operating procedures, and training standards to ensure similar problems cannot happen in other areas of the business.


D8: Recognize Team and Individual Contributions


Formalize the closure of the project and celebrate the team's success.
